shoulder dislocation..

DISLOCATION OF THE SHOULDER

MECHANISM
• A FALL ON AN OUTSTRETCHED HAND WITH THE SHOULDER ABDUCTED AND EXTERNALLY ROTATED
• DIRECT FORCE PUSHING THE HUMERUS HEAD OUT OF THE GLENOID CAVITY
• DIRECT BLOW ON THE FRONT OF THE SHOULDER DRIVING THE HEAD BACKWARDS

CLASSIFICATION
- Anterior dislocation
- Posterior dislocation
- Luxatio Erecta

PATHOLOGICAL CHANGES
- Bankart's lession
- Hill - Sach lession
- Roundening off the anteriro glenoid rim
- Associated fractures of greater tuberosity of the humerus

DIAGNOSIS
- Presenting complaints : PT enters casualty with shoulder abducted and the elbow supported with the opposite hand.
- History of fall on an outstretched hand followed by pain and inability to move the shoulder

PHYSICAL EXAMINATION
- Pt keeps his arm abducted
- Normal round contour of shoulder joint is lost
- Fullness below clavicle due to displaced head
- Apprehension test
- Fulcrum test
- Drawer test
