Colle's fracture ; example about principles of management


COLLES’ FRACTURE
Treatment :
Undisplaced fractures
- Cast application and immobilisation for 6 weeks in adults and 3-4 weeks in children
Displaced fractures
- Closed reduction and cast application- 6 wks-adults
- Closed reduction and percutaneous fixation

Closed reduction and cast application
- Under sedation / ga
- Traction/counter traction
- Fracture manipulated into position after closely studying the x-rays for direction and amount of displacements / angulation
- Position of immobilisation- Cotton-loader position- palmar flexion and ulnar deviation at the wrist
- Below elbow cast applied and check x-ray done to confirm acceptable position care of cast and follow rules of / during immobilisation – physiotherapy started – other joints
- Check x-ray after a week to confirm position. Re-reduce if position is lost
- Immobilise for 4-6 weeks
- Later mobilise

Closed reduction & percutanous fixation
Indicated
- When the fracture is found to be very unstable during closed reduction
